Background:
HCV continue to pose significant public health challenge in northern Germany. The incidence of HCV also increases substantially, rising from 202 cases in 2021 to 313 cases in 2022, indicating a persistent and concerning trend.
Description of model of care:
In alignment with efforts to eliminate HIV and improve access to treatment, Aidshilfe Schleswig-Holstein has expanded its low-threshold HIV and HCV testing services to better reach key populations across the federal state, covering 15,800 kilometres2 and serving a population of 2.9 million residents. Testing activities are conducted in 26 cities across eight regions, reactive results are immediately linked to care.
These services are delivered through Check-Mobil, a van designed to facilitate on-site testing. According to a set schedule, Check-Mobil travels to various locations where people who use psychoactive substances live or spend time, such as public parks, urban gathering spots, and social service facilities that provide support to this population.
All testing services are voluntary, anonymous, and free of charge, ensuring accessibility and confidentiality. The initiative offers a comprehensive package of services, including:
•Comprehensive pre- and post-test counseling
•Multilingual educational materials on HIV, STIs, and HCV
•Provision of condoms and other preventive resources
•Distribution of harm reduction materials.
•Fentanyl testing services to mitigate the risks associated with opioid use
Effectiveness:
In 2024, the Check-Mobil initiative provided testing and support to 659 individuals (62.8% male, 37.2% female), administering 329 HIV tests and 330 HCV tests. No HIV-positive cases were detected, while 20 individuals tested reactive for HCV, resulting in a positivity rate of 6.06%.
Conclusion and next steps:
The initiative demonstrated notable effectiveness and outreach, successfully engaging populations with limited healthcare access. These findings underscore the importance of maintaining and expanding such programs throughout Schleswig-Holstein to address persistent health inequalities and improve public health outcomes.
